Methods for making a FOIA request:

You may request the information and records available to the public in the following manner:
-Use request form CLICK HERE.
-The request should be directed to: Cathy Palmer, Executive Director and FOIA officer
-Your request will be responded to within five (5) business days, if possible. An extension of an additional five working days may be necessary to properly respond.
-You must indicate whether you have a “commercial purpose” in your request.
-You must specify the records requested to be disclosed for inspection or to be copied. If you desire that any records be certified, you must specify which ones.

Fees for Copies:
-First 50 pages of black and white, letter or legal paper are free
-After the first 50 pages the library charges $0.15 per page
-Colored copies and copies sized other than legal or letter size will have a $0.50 per page fee
-If the records are kept in an electronic format, you may request a format and if feasible, they will be provided, but if not, they will be provided in the electronic format in which they are kept (and you would be required to pay the actual cost of the medium only) or in paper as you select.
-Certified copies are $1.00

*The place and times where the records will be available is the Mokena Community Public Library District Administrative Offices (11327 w. 195th St., Mokena, IL) from 9:00 AM to 5:00 PM, Monday through Friday, excluding holidays.
